What is a sliding fee scale?

A sliding fee scale allows the fee or co-pay for health care services to be based on a household’s income.

What will I pay under the program?

The sliding fee is based on the Federal Poverty Level guidelines. Use the calculator below to see your estimated fee based on your household income.

How do I apply for this program?

To apply, simply bring one of the approved documents to prove annual income to your next office visit.

How will I know that if I am approved?

Patients who complete the application process for the Sliding Fee Scale Program will be notified within minutes. If you are denied, you are welcome to reapply at any time.

How often do I need to renew my status?

Sliding Fee Scale discounts are good for one year. Patients must reapply to continue discount.

How do I qualify?

To qualify for the Sliding Fee Program, patients must show proof of gross annual income for all immediate family members living in their household within 21 days of their first visit. Gross income is all income from all sources before taxes.

Bring ONE of the documents below to prove income:

• Prior year tax return
• Most recent 4 weeks pay stubs.
• Letter from employer stating patient’s income. SHC would prefer document be on letterhead and must include employer’s name, address and phone number.
• Social security letter for fixed incomes such as social security, disability, pension, etc.
• Most recent unemployment compensation documentation.
• Letter of reference from a 501(c)(3) non-profit organization. SHC would prefer document be on letterhead and must include employer’s name, address and phone number. (ex. Church, United Way, etc.)
• If none of the above information is available, a patient may self-declare
