A sliding fee scale allows the fee or co-pay for health care services to be based on a household’s income.
The sliding fee is based on the Federal Poverty Level guidelines. Use the calculator below to see your estimated fee based on your household income.
To apply, simply bring one of the approved documents to prove annual income to your next office visit.
Patients who complete the application process for the Sliding Fee Scale Program will be notified within minutes. If you are denied, you are welcome to reapply at any time.
Sliding Fee Scale discounts are good for one year. Patients must reapply to continue discount.
To qualify for the Sliding Fee Program, patients must show proof of gross annual income for all immediate family members living in their household within 21 days of their first visit. Gross income is all income from all sources before taxes.
